(1) Vendor needs to provide Life Insurance, Long-term Disability Insurance and Flexible Spending Accounts Administration Services for its benefits eligible employees for fiscal year-end financial accounting reconciliation information shall include:
- Paid premium for the period, Paid claims for the period, separate listing of all claims, full accounting of all reserves, Date of injury/illness and reporting date, Social security/rehabilitation status, Gross monthly benefit, Identification of offsets, Net monthly benefit and Reserve by claimant, etc.
(2) All questions must be submitted no later than January 28, 2021.

Onshore (USA Only);

[D] Work Performance:

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
