(1) Vendor needs to provide occupational health services to the government authority located in Illinois.
- Fitness-for-duty Examinations - The physician will be responsible for evaluating employees following non-work-related injury, illness, extended leave, or other situations that may raise questions of fitness-for-duty
- Workplace Injury Evaluation – The physician will be responsible for evaluating employees following non-emergent work place related injuries and illnesses to determine fitness-for-duty, treatment plans, etc.
- The physician will determine if the employee is fit to return to full or limited duty
(2) All question must be submitted no later than January 25, 2021.
